I herby authorize the repair work herein set forth to be performed with the necessary materials and agree that Beck & Masten Repair Center is not responsible for the loss or damage to the vehicle in case of fire, theft or causes beyond our control or for any articles left in any vehicle.  Beck & Masten Repair Center will not be held responsible for any delays caused by the unavailability of parts.  I give permission for Beck & Masten Repair Center to operate the vehicle herein described on streets, highways, or elsewhere for the purpose of testing and/ or inspection.  An expressed mechanic’s lien is herby acknowledged on the above vehicle to secure the amount of repairs hereto.  Storage fee charges commence one day after notification of completed repairs.  Should the work agreement be cancelled by either party, the following fees may be applied; storage fee($30.00 per day) , administrative fee ($75.00), restock fee, tear down fee & transfer fee, which shall be charged from the date of drop off to the time the vehicle is delivered.  Beck & Masten Repair Center is not responsible for any damages from freezing and/ or overheating due to lack of antifreeze or coolant.  Parts prices are subject to change without notice.  If your vehicle has an original estimate of over $3000.00 the repairs may take over 30 days to complete.  You may need to make arrangements for alternate transportation in the event your insurance company allows 30 days for rental.  Should you wish to exercise your warranty in the future, please be advised that Beck & Masten Repair Center will not be responsible for alternate transportation.  This document will also serve as a “Power of Attorney” for endorsements of payments or documents of said vehicle.  Promise time is estimated time only.  The total amount of repairs are the customers responsibility and are due in full at the time of pickup.
